Insurance and Payment Policy
Without a current referral or current medical coupon, we will need to reschedule your appointment unless prior arrangements have been made with our office.
Our fee structure is based on standards within both the community and our specialty. Fees vary with the severity and complexity of the condition. Please remember, insurance companies frequently set allowable fees below our customary charges. We have participating agreements with several insurance companies and honor their fee schedules.
If your insurance company requires a co-payment for your office visit, we will collect it at time of service. Along with any co-payments, we will also collect your co-insurance amount if surgery is scheduled.
While we are contracted with several insurance companies, we are happy to bill any insurance company as a courtesy to our patients.
Cascade Surgical Partners has a contract with the following insurance companies:

- Greatwest Healthcare
- Group Health
- Medicare (Accept Assignment)
- Molina Health Care
- Trusteed Plans
If you have do not have medical insurance, a surgical deposit will be required to schedule any non-emergent surgeries. We offer many payment options, including a "quick pay" discount for those patients without insurance. Our billing department is available to answer any questions regarding our insurance and payment policy. They can be reached during normal business hours at 509-575-3946.
